Elderly and frail individuals
This is a project aimed at promoting the anticipation and coordination of care for frail elderly people - over 65 years old - in the canton of Geneva.
The Geneva University Hospitals, Geneva Doctors and Imad launched a pilot using the COLNEC platform :
- 1 year
- 400 patients
- 180 healthcare professionals
Patients in complex situations
The Intermed project is an interprofessional project that takes place in a primary care practice, the Medi-Centre in La Chaux-de-Fonds. It develops a model optimising patients'care, particularly for those dealing with with more chronic diseases, in complex situations. These patients are generally multi-morbid and in unstable bio-psychosocial situation.
The research project allowed the development of a ``shared care plan`` in order to :
- facilitate communication between professionals and free up medical time
- improve care coordination
- identify patients at risk of developing a complication or additional frailty
Diabetic and heart failure patients
Colnec participated in the “Telemedicine Experiments for the Improvement of Health Care Pathways” (50 patients) in France.
- The COLNEC V2 platform, certified as a class 1A medical device, was used in the ETAPES telemonitoring programme.
- Colnec has been recognised compliant with the recommendations of the DGOS, the French Ministry of Public Health, on remote medical monitoring.
- The solution enabled more effective monitoring of patients through health and quality of life indicators, while improving TPE and patient compliance.
